Survivors may be eligible for coverage. Survivors failing to elect continued coverage within the time-frames specified below may not enroll at a later date. Coverage will end the last day of the month of the subscriber's death.
- Active Non-Vested Subscriber
MCHCP will notify survivor of the right to continue coverage. Survivor must elect coverage within 60 days of notice. Covered dependents are eligible for up to 36 months of coverage under COBRA. The initial premium payment for COBRA coverage must be received within 45 days of election, and then MCHCP will bill monthly.
- Active Vested Subscriber
Survivors covered at the time of the subscriber’s death may continue coverage. Survivors not currently enrolled may enroll by providing:
- Proof of other coverage for six months immediately prior to subscriber's death; and
- Proof of eligibility for dependents
Submit a completed Survivor Enrollment Form and pay the required premium within 31 days of the first day of the month after the subscriber’s death.
- Terminated Vested or Retired Subscriber
Coverage for Medicare primary survivors is automatically continued. MCHCP will send notification of coverage and premium.
Non-Medicare survivors covered at the time of the subscriber’s death may continue coverage. Submit a completed Survivor Enrollment Form and pay the required premium within 31 days of the first day of the month after the subscriber's death.

Dependents may be enrolled in subscriber’s current plans at a later date within 31 days of a life event (Marriage, Birth or Adoption [or placement of adopted child]) or 60 days of Loss of employer-sponsored group coverage with proof of 12 months of coverage immediately prior to the loss.
