Qualified Medical Child Support Order (QMCSO)

Submit completed Enroll/Change/Cancel Form and QMCSO within 60 days of court order. Coverage becomes effective the first of the month after receipt of enrollment and QMCSO, or on the date specified by the court. If the enrollment and QMCSO are received on the first day of the month, coverage is effective immediately.

Active subscribers who fail to enroll the child during the 60-day period can enroll during the next Open Enrollment for coverage effective the following January.
