Active Employee
For absences fewer than 31 days, coverage continues as if subscriber has not been absent. Otherwise, coverage stops the last day of the month in which military leave begins unless subscriber elects to pay for COBRA coverage at the COBRA rate. If subscriber does not elect to continue coverage for eligible dependents, it is suspended effective the last day of the month in which military leave begins. If utilizing annual and/or comp-time balances to stay on payroll, the dependent coverage is at the active employee rate.

Submit completed the online "Return to work from leave of absence" eForm (or a completed Enroll/Change/Cancel Form) within 31 days of return to work to reinstate the same plan and level of coverage as previously enrolled. When the subscriber returns to work, coverage may be reinstated effective the first of the month, the first of the following month, or the first of the month after the loss of military coverage (with proof of loss).

Subscribers who fail to reinstate coverage within the 31-day period may enroll during Open Enrollment for coverage effective the following January.

Retiree Subscriber
MCHCP coverage may be suspended while on active duty. Submit completed Enroll/Change/Cancel Form and a copy of active duty orders. If coverage is suspended for subscriber, dependent coverage also is suspended.

When military deployment ends, submit completed Enroll/Change/Cancel Form and an official document indicating the separation date within 31 days of discharge to reinstate the same plan and level of coverage as previously enrolled. Coverage may be effective the first of the month, the first of the following month, or the first of the month after the loss of military coverage (with proof of loss).

Retiree subscribers who fail to reinstate coverage within the 31-day period cannot enroll at a later date.
