Once an employer notifies MCHCP of an approved leave of absence, a letter, Leave of Absence Enrollment and bill (if applicable) will be mailed. To continue coverage, submit the online "Leave of Absence" eForm (or a completed Leave of Absence Enrollment Form) and submit payment (if applicable) to MCHCP within 14 days. Subscriber will be billed at the Leave of Absence rate on a monthly basis until they return to work. To determine the rate, use the premium calculator. If not continued, coverage ends the last day of the month of active employment or premium was paid in full.

Subscriber may choose to maintain employee-only coverage while on leave and suspend coverage on dependents. Subscriber’s coverage may also be suspended while on leave. Coverage may be resumed for all members upon subscriber’s return to work by submitting the online "Return to work from leave of absence" eForm (or a completed Enroll/Change/Cancel Form) within 31 days of return.

To cancel coverage, notify MCHCP immediately by phone or by submitting a Leave of Absence Enrollment. Coverage ends on the last day of the month in which the request to cancel is received by MCHCP or premium was paid in full.

Upon return to work, if coverage was continued during leave, paycheck premium deductions will resume at the active employee rate.

Suspended coverage may be resumed by submitting the online "Return to work from leave of absence" eForm (or a completed Enroll/Change/Cancel Form) within 31 days of return to work. Once an employer notifies MCHCP of subscriber’s return to work, a letter and enrollment form will be mailed. Coverage becomes effective the first of the month following receipt of the enrollment, in the same plan and level of coverage as previously enrolled.

If spouse is an MCHCP subscriber (active or retired), coverage may be transferred to the plan in which spouse is enrolled. Spouse must submit the online "Add coverage due to loss of employer-sponsored or MCHCP group coverage" eForm (or a completed Enroll/Change/Cancel Form) to MCHCP as soon as possible.
