Adoption

Submit completed enrollment form within 31 days of event date. Coverage becomes effective on the date of adoption or placement for adoption. Event dates include:

• Date of birth (If petition for adoption is filed within 31 days of child's birth date)
• Petition for adoption
• Order of placement for adoption
• Final adoption decree

MCHCP reserves the right to request proof of eligibility.

Advise MCHCP of child's Social Security number verbally or in writing as soon as it is received. Failure to provide proof of dependent's Social Security number in a timely manner may result in loss of coverage.

Active subscribers who fail to enroll during the 31-day period may enroll during the next Open Enrollment for coverage effective the following January.

Retiree subscribers who fail to enroll the child during the 31-day period cannot enroll them at a later date.
