Hearing aids are covered for conductive hearing loss unresponsive to medical or surgical interventions, sensorineural hearing loss and mixed hearing loss. Prior to receiving a hearing aid members must receive:

  • A medical exam by a physician or other qualified provider to identify any medically treatable conditions that may affect hearing; and
  • A comprehensive hearing test to assess the need for hearing aids conducted by a certified audiologist, hearing instrument specialist, or other provider licensed or certified to administer this test.

Covered one hearing aid, per ear, every two years. Replacement hearing aid batteries are not covered. If hearing aid cost exceeds the amount listed below, member is also responsible for charges over that amount.

  • Conventional: $1,000
  • Programmable: $2,000
  • Digital: $2,500
  • Bone Anchored Hearing Aid (BAHA): $3,500

Non-Medicare members enrolled in a UMR plan may receive additional discounts by using an Ear Professionals International Corporation (EPIC) provider. Contact EPIC at 866-956-5400 for more information.
